※留学生センター所属の学生は優先的にこのプログラムに履修登録できます。
※履修が決定した学域正規生には、下記２種類の申込用紙を配布します。

【注】ほとんどの正規生は入学時に 4 年分加入しているので、新たに加入する必要はありま
せん。自分が現在加入しているかわからない人は留学生係で確認してください。
Please be sure to attend the orientation meeting.
※Students who can enroll in the course will be determined at the orientation meeting. If
they do not attend the orientation they are not eligible for the course.
※Students who are affiliated to the International Student Center have priority of other
students on registration.
※Application forms for two kinds of insurance will be distributed to the students who are
to enroll in the course. They are:
1. “Personal Accident Insurance”: 1000 yen / year
2. “Liability Insurance”: 340 yen /y ear
(Students who are affiliated to the International Student Center will receive those forms
at the program orientation.)
保険料（２種類）を郵便局で振り込んでください。
※２種類（学研災と学研賠）の領収書のコピーがなければ履修はできません。
大学内郵便局の営業時間（窓口）9：00am～4：00pm（地図 N1）
Pay for 2 kinds of insurance at the post office.
※ Copies of the payment receipts must be submitted to the International Students
Center at the Administrations Office with application form.
Office hours of the Post Office on campus: 9：00am～4：00pm (MAP N1)

「日本文化・社会体験プログラム」履修申請書と２種類の保険のコピーを提出してください。
提出期限 ：２０１４年１０月１５日（水）午後５：００まで
提出場所 ：留学生センター （本部棟４階、 地図 C1）
※提出期限は厳守してください。
Submit the copies of the payment receipts with the registration application form for the
"Experiences in Japanese Culture and Society Program"
Submission Deadline: October 15th, 2014 (Wed) before 5:00pm
Where to submit: International Student Center - 4 Floor, Administration Office (MAP C.01)
