What Kind of Place Is a Day Care Center? A new system for child and childcare support will start in April 2015. With the start of this system, when wishing to use authorized daycare centers or the Childcare Mom Service, parents need to have a certificate issued by the city government. *Information on the procedures necessary to receive a certificate from the city government, as well as information on available childcare days and facilities, will be announced in in the fall of 2014. Day care center is a facility where infants and young children are looked after if their parents cannot take care of them at home due to work or disease. Injury/Disease Prevention ♦♦♦Prevention of child accidents♦♦♦ The leading cause of death among children is unexpected accidents. Such accidents can be prevented by taking precautions and care. Accidental ingestion Infants and young children put anything into their mouths. The size of their mouths is said to be 39 mm at a maximum, and anything smaller than this can be swallowed. Accidental ingestion becomes frequent from around the age of six months. Keep cigarettes, detergents and anything else that can be swallowed by children out of their reach (at least 1 m above the floor). Fall Infants and young children are not good at maintaining their balance due to their relatively large and heavy heads. They still have a narrow field of vision, so they often trip over things or fall down from a height. Do not place anything that could serve as a stepstool near windows or on the balcony. Near-drowning Infants and young children could drown in as little as 10 to 20 cm of water. Especially children aged one year often have accidents in the bath. Keep an eye on them while they are bathing, devise a way to prevent them to enter the bath by themselves and let the water out of the bath. Do not place anything that could serve as a stepstool around the washing machine. Burns Infants have a thin skin, so they get burned by being exposed to a low temperature for a short time. They are also very curious; therefore, touch heaters, hot water/steam and other hot items, resulting in burns. Keep anything hot out of children’s reach. Do not use a table cloth since they often burn themselves by pulling the cloth and spilling something hot on the table onto themselves. ♦♦♦SIDS (sudden infant death syndrome) ♦♦♦ SIDS is a disease resulting in the sudden death of a healthy infant, which is not caused by an accident or suffocation, but occurs while the baby is asleep. It occurs most often among infants aged two to six months, and most victims are babies under the age of one year. The cause of SIDS is unknown, but some factors that can increase the risk have been identified. To reduce the risk of SIDS (1) Put infants on their back to sleep, (2) Refrain from smoking as a family, and (3) Breastfeeding Sleeping in the prone position, smoking and artificial milk feeding are not directly responsible for SIDS, but the incidence of the syndrome might be reduced by avoiding these factors. It is also important not to leave a baby alone. 38 13. First Aid for Children Accidental ingestion (the treatment varies according to the swallowed object.) 1. Check if it is something that the child should vomit (see the table below). 2. If it is not something that the child should vomit, take him or her to a medical facility immediately. 3. If it is a thing that the child should vomit, remove the remaining object from the mouth with your fingers. After giving him or her one or two glasses of water or milk, put your finger in the child’s throat and push on the base of the tongue to get the child to vomit. *Take the swallowed object or the vomit along to the medical facility. Do not panic, loosen the clothes and turn the face to the side to let the child lie down quietly. 2. If the child has a fever, use a cold towel to cool him or her. 3. Convulsions usually subside within five minutes, but time the convulsions. *You don’t have to prize the mouth open with a spoon or something, or have the child bite a chopstick. Burns 1. Completely cool the burns as soon as possible using tap water or clean running water. Do not remove the clothes but cool the child with the clothes. 39 2. Be careful when using ice or ice packs. If they are used for a long period of time to cool the affected area, it may become too cold and get worse. 3. If the burn is extensive, do not cool it for longer than 10 minutes to prevent the entire body from getting cold. Choking In case the sick child responds, 1. In case of choking, immediately ask someone around to call the emergency services (119) and try to remove foreign substances from the mouth. 2. Place the child on your arm, position him or her on the stomach with the upper body at a lower level and give several blows to the center of the child’s back with your hand while stabilizing the lower jaw of the child with your other hand. *If the child is quite big, put him or her on his/her side and give hard blows between the shoulder blades with the palm of your hand. In case the sick child does not respond, immediately perform cardiopulmonary resuscitation. Near-drowning 1. Pull the child out of the water quickly. 2. Clear the child’s airway if there is no response (unconscious), and provide cardiopulmonary resuscitation if the child is not breathing normally. Do not put pressure on the abdominal part. ♦♦♦Cardiopulmonary resuscitation♦♦♦ Chest compression (cardiac massage) In case of an infant (under one year old): Provide 30 chest compressions to an infant at the rate of at least 100 times per minute. Place your two fingers (index and middle fingers) on the chest, slightly below the line connecting the nipples, and press down approximately one-third the depth of the chest. In case of a young child (aged 1 to 7 years): Use one or both hands and compress the chest 30 times at the rate of at least 100 times per minute at the center of the chest (the center of the line connecting the nipples). Press down approximately one-third the depth of the chest. 40 Artificial breathing 1. If the child is under one year old, perform mouth-to-mouth/nose rescue breathing. 2. Cover his or her mouth and nose all together with your mouth. 3. Blow air twice, one second at a time, and check if the child’s chest is rising. Continuity of cardiopulmonary resuscitation Continue with 30 chest compressions and two breaths until a rescue crew arrives. If it is difficult to perform artificial breathing, artificial respiration alone is also effective. How to use AED 1. Press the power supply button and follow the voice message instructions and flashing lamps on the main unit. 2. Continue chest compression if possible until the unit becomes operative. 3. For an infant or preschool child, use special electrode pads for children if they are available. *If not available, use those for adults. 4.
