Your National Health Insurance card certifies that you are a member of the Taito
City National Health Insurance system. Submit your National Health Insurance
card without fail when you receive treatment at a hospital or medical institution.
Although NHI membership is in units of one household, a health insurance card
is issued to each individual member.
国民健康保険被保険者証（保険証）は、台東区の国保に加入していることを証
明します。病院や診療所の診療を受けるときは、必ず保険証を提出してください。
国保は世帯ごとの加入ですが、保険証は加入者 1 人につき 1 枚発行します。

1) Confirm details: The National Health Insurance card will be mailed to the
household after joining. When you receive your card, check that the information is recorded correctly.
*Make sure your name is clearly visible on your mailbox to ensure delivery.
2) Recording changes: If any of your personal information is recorded incorrectly, or if there has been a change in the details on the card, make sure to
notify the National Health Insurance Section. Your card will be invalid if
you make any corrections yourself.
3) If you are no longer eligible: If you have become a member of another
Japanese public health insurance system, moved out of Taito City or are
leaving the country, you may not use the health insurance card issued by
Taito City. If you use the card, you will be required to reimburse Taito City
for the fees it paid.
4) When the card expires: The National Health Insurance card is invalid after
the expiration date recorded on it. If you move out of the city of Taito, the
health insurance card becomes invalid, even if it is before the expiration
date. You must be sure to return the invalid health insurance card.
5) Improper use: It is illegal to lend your National Health Insurance card to
another person or to use another person’s National Health Insurance card. If
you do so, you may be liable for prosecution.
① 内容を確かめて：新規加入世帯の保険証は郵送します。保険証を受けとっ
たら、氏名などに間違いがないか、よく確かめてください。
★郵便物が届くよう、ポストには必ずお名前を表示してください。
② 訂正するときは：氏名などに間違いがあったり、内容に変更があったときは、
必ず届け出てください。自分で書き直すと無効になります。
③ 資格がなくなったら：他の日本の公的健康保険に加入したり、区外へ転出
した（出国する）ときは、台東区の保険証は使えません。もし使ったときは、
区で負担した医療費は返していただくことになります。
④ 有効期限が過ぎたら：保険証に記入してある有効期限が過ぎますと、その
保険証は無効となります。有効期限内でも転出などで使えなくなった保険
証は必ずお返しください。
⑤ 不正使用すると：保険証を他人に貸したり、他人の保険証を使うと、法律
により罰せられる場合があります。

주십시오 .
국보는 세대별로 가입하지만 , 보험증은 한 사람당 1 개씩 발급됩니다 .

① 确认内容 : 向新加入家庭寄送保险证。收到保险证后，请确认姓名有
无差错。
※ 请务必在自家信箱上书写姓名，以便邮局邮递。
② 订证时 : 姓名若有差错或内容有所变更时，请务必申报。自己改写一
概无效。
③ 丧失资格时 : 加入其他日本的公共健康保险，或者迁出区外 ( 出境 ) 时，
台东区的保险证不能继续使用。如果发现继续使用时，则要求退还
由区里负担的一切医疗费。
④ 过期时 : 保险证上所记载的有效期过后，保险证无效。在有效期内迁
出等而不能继续使用的保险证请务必退回。
⑤ 使用不当时 : 把保险证借给他人，或者使用他人之保险证时，将受到
法律制裁。

전출 ( 출국 ) 할 때는 타이토구의 보험증은 쓸 수 없게 됩니다 . 만일
계속 사용하게 되면 나중에 구가 부담한 의료비를 반납해야 합니다 .

유효기한이 지나면：보험증에 기재되어 있는 유효기한이 지나면 그
보험증을 무효가 됩니다 . 유효기한내일 때도 전출 등으로 못쓰게 된
보험증은 반드시 반납하여 주십시오 .

부정하게 보험증을 사용하면 : 보험증을 남에게 빌려 주거나 타인의
보험증을 사용하는 등의 부정행위를 하면 법률에 따라 처벌받을 수
있습니다 .
