妊娠届
港区に在住する人で、医療機関で妊娠の診断を受けた人は、区役所に届け出てください。
「母子健康手帳」と「母と子の保健バッグ」およびファイルを差し上げています。このバッ
グとファイルの中には母親学級のお知らせ、両親学級のお知らせと妊婦健康診査受診票な
どが入っています。都内の委託医療機関に妊婦健康診査受診票を提出すると、受診票記載
の検査項目が公費負担の対象となります。
また、妊娠高血圧症候群などにかかっている妊産婦が入院する必要がある場合は、医療
費の助成を受けることができます（所得制限あり）。
※ 医療費助成の申請は、退院日から 3 カ月以内に申請してください。

Childbirth
Pregnancy Notification
Once a resident of Minato City confirms that she is pregnant by undergoing a
medical facility health checkup, she should notify City Hall of her pregnancy. At this
time, she will be given a Maternal and Child Health Handbook, a Maternal and Child
Health Bag, and a file. The bag and file contain information on maternity and parenting classes, and a pregnancy health checkup form, etc. If the mother-to-be presents
a pregnancy health checkup form to a participating medical facility in the Tokyo
Metropolitan Area, she can receive the checkup stated on the coupon free of charge.
If hospitalization is required due to pregnancy-induced hypertension, a medical
expense subsidy is available but income restrictions apply.
*Apply for a medical expense subsidy within three months of hospital discharge.

妊婦健康診査の費用助成
都内の委託医療機関であれば、受診票に記載された検査項目が公費負担の対象となりま
す。各総合支所区民課保健福祉係の窓口で、母子健康手帳申請時に併せて受診票をお渡し
します。都外医療機関または助産院で妊婦健診などを受診した場合の費用助成受診票は、
都外医療機関や助産院（助産院は都内・都外ともに）では使用できないため、里帰り出産
などで都外医療機関や助産院で妊婦健康診査を受診した場合に、出産後申請により費用の
一部を助成します。詳しくは、母子健康手帳とともにお渡しするご案内と申請書をご覧く
ださい。

出産への補助
港区では、経済的な理由により入院して出産することができない状況にある妊産婦に対
し、指定助産施設での出産支援を行います。

出産費用の助成
出産費用から健康保険組合などの出産育児一時金などを差し引いた全額（限度額あり）
を助成します。公的医療保険に加入している保護者が出産前から港区に住み、生まれてく
る子どもも出生日から港区に住むことが条件です。
助成する金額は、出産費用（上限 60 万円）から出産育児一時金などを差し引いた額です。
所得額による制限はありません。
※ 申請は出生日から 1 年以内に申請してください。

Subsidy for Prenatal Health Checkup
If the mother-to-be presents the checkup-form to a designated medical facility
in the Tokyo Metropolitan Area, she can receive the checkup stated on the coupon
free of charge. Health checkup coupons are distributed with the Maternal and Child
Health Handbook at Health and Welfare Subsection of Residents Support Section,
Regional City Office. When an expectant has a checkup at a medical institution outside Tokyo or a maternity center in/outside Tokyo, the checkup form cannot be used.
Apply the subsidy after she gives birth, then Minato City will subsidize the part of
the cost. For details, see the information and the application provided along with your
Maternal and Child Health Handbook.

Assistance with Childbirth
Minato City provides childbirth assistance at designated birthing facilities to pregnant women who are unable to be hospitalized for childbirth for financial reasons.

Subsidy for Childbirth Expenses
If your childbirth expenses exceed the lump-sum childbirth allowance paid by your
health insurance, Minato City will subsidize the difference up to a certain amount. To
receive this benefit, guardians need to become members of public health insurance,
have been residing in Minato City before the birth of their baby, and also are planning to raise their child in Minato City from the child’s day of birth.
The amount of the subsidy is the balance due on childbirth expenses up to
¥600,000 after deducting the lump-sum childbirth allowance, etc. There are no
income restrictions.

乳幼児の保健
港区では、3・４カ月児、6・7 カ月児、9・10 カ月児、1 歳 6 カ月児を対象とした健康
診査をいずれも委託医療機関で実施し、３歳児健診（内科・歯科）、１歳６カ月児、就学
前の乳幼児・妊産婦を対象とした歯科健診をみなと保健所で実施しています。費用は無料
です。
港区委託医療機関については、港区ホームページをご覧ください。なお、保健師・管理
栄養士・歯科衛生士による育児相談を行っています。

育児のための手当
児童手当

子どもの疾病への医療費助成制度
子ども医療費助成
中学 3 年生まで（15 歳に達した日以後の最初の 3 月 31 日）の子どもの通院および入院
に係る医療費の自己負担分を助成します（保険適用外を除く）。医療費の助成を受けるた
めには、所定の用紙に必要書類を添えて申請してください。都内の医療機関に健康保険証
と医療証を提示すれば、原則として自己負担分を支払わずに診療、薬の処方が受けられま
す。

Health for Infants
Minato City provides health checkup through partner medical facilities for infants
at the ages of 3-4 months, 6-7 months, 9-10 months, and 18 months. A 3-year health
and dental checkup, as well as dental checkup for 18-month-old and preschool aged
children and pregnant women are also provided at Minato Public Health Center.
These checkups are free of charge.
For the list of designated medical institutions in Minato City, visit the city’s website. The centers also offer child-rearing consultations provided by public health
nurses, registered dieticians, and dental hygienists.

Subsidy Programs for Children’s Medical Care
Medical Expense Subsidy for Children
This subsidy helps cover the out-of-pocket medical expenses incurred for in-patient
or outpatient hospital care provided to children through their third year of junior
high school until the first March 31 after their 15th birthday (excluding procedures
not covered by health insurance). To receive this medical expense subsidy, attach the
necessary documents to the prescribed application form and submit it to the office
indicated. If you show your health insurance card and medical care certificate to a
medical facility in the Tokyo metropolitan area, you can generally receive medical
treatment and prescriptions without paying your out-of-pocket portion.

Child-rearing Medical Care
This subsidy pays for medical expenses incurred when a child with an orthopedic
impairment; a sight; hearing; equilibrium; vocal; speech; or eating impediment or a
child who requires the surgical treatment for coronary disease; kidney disease; congenital visceral disease; liver dysfunction, is treated at or admitted to a designated hospital.
